How to Become a New Patient

Becoming a new patient is simple.


Call our office or request an appointment.

Our staff will help guide you through the process and answer any questions you may have.

 

Before your first visit, please complete the required online forms on our Patient Forms page so we can best prepare for your child’s care.

What to Bring to Your First Visit

To help us create a complete medical record for your child, please bring:

​

• Insurance card
• Parent or guardian photo ID
• Any medications or supplements your child takes
• Shot records or prior medical records (if available)
• Completed forms from the Patient Forms page

 

You will receive a text message the day prior to your appointment that allows you to confirm your visit and complete pre-registration. This is our preferred method of registration.

​

If your child is transferring from another practice, please complete a Medical Records Release so their records can be sent to us.

When to Choose a Pediatrician

We recommend selecting a pediatrician during your third trimester. This gives you time to feel confident in your choice and ensures a smooth transition once your baby is born.

After Your Baby Is Born

Once your baby arrives, simply call our office at (586) 558-2111 to schedule your newborn’s first visit.

​

Newborns are typically seen within the first few days after hospital discharge so we can check weight, feeding, and overall health.

Hospital & First Visits

If you deliver at a local hospital, the hospital team will guide your baby’s initial care. After discharge, we will continue that care in our office and support you through those early weeks.

Adding Baby to Insurance

Be sure to contact your insurance provider shortly after birth to add your baby to your plan. This helps avoid any delays in coverage for early visits.
